NOMINATION FORM

Before making the nomination, please make a note of the following instructions:

  • Only companies based in the UAE can be nominated for EHA - UAE 2025
  • All fields in the submission form are mandatory and must be filled with correct information
  • Companies can nominate in upto 3 categories only. Nominations are free for 2 company categories.
  • There will be a nomination fee of USD 100 charged for the 3rd company category nomination
  • The online submission form has 2 pages. Page 1 requires you to submit basic information and select the categories you wish to nominate in. Page 2 requires you to share a brief on why your company deserves to win in the selected categories. You can refer to a sample PDF for responses. Please note that responses on Page 2 can also be submitted later.

  • Highly Effective: Exceeds expectations in addressing this area; outcomes and experiences are outstanding.
  • Effective: Meets expectations; consistently performs well in this area with few areas for improvement.
  • Moderately Effective: Shows progress but has room for significant improvement to meet expectations.
  • Somewhat Effective: Addresses some aspects but lacks consistent or comprehensive efforts in this area.
  • Not Effective: Fails to adequately address this area; significant gaps or issues remain.
